He can only be engaged when (nearly) all of the NPC's in zone have been killed. If this has not happened, he will despawn upon engage. Once he has been killed, he will spawn several chests, one of which includes [[Epic|Completed Epic 6.0]]. He always drops a single [[essence]] of Anguish.
